A stock market index is a composite value of a listing of stocks in a stock market exchange. Some of the global stock market indices you can trade on include the FTSE 100, German Dax 30, French CAC 40, Japanese Nikkei 225 and the three US indices ie Dow Jones 30 (also known as Wall Street), S&P 500 and the Nasdaq 100.
To spread bet on stock market indices you do so in the same way as other spreads bets. The spread bettor is given a spread or quote, they bet on the movement of the stock market indices. Based on the direction and movement of the market, the bettor either makes profits or incurs a loss.
A number of big spread betting firms offer both a daily and a weekly Index market. Some markets eg the NASDAQ 100 have greater unknowns and therefore can be more volatile than markets like the FTSE 100. If you trade a daily bet but want to keep it for longer, sometimes you have the option to 'roll' the bet over into the next trading day. But despite the tighter bid-offer spread for shorter expiry dated spread bets, this can become expensive if you wish to hold onto them.

Of course there are many factors that influence the movement of stock market indices. Naturally share prices are influenced by takeover news (and rumour), company results, company employment data, historic performance, dividends, political events etc. When the shares in a stock exchange move, the stock market index fluctuates accordingly.
